What we do

All visits are person centred. This means we work with you to decide on goals and make a plan for your care.

Some of the things we can help with include:

  • Identifying and meeting complex physical and mental health needs

  • Signposting to other services if we’re not the right service for you

  • Helping you to understand your health and well-being

  • Understanding and expressing feelings and emotions

  • Working with other services such as your GP and social services

  • Our work focuses on improving your quality of life

More details about the support that can be provided by each different health professional are available on their pages.

We are a community team

This means we can visit you:

  • at home
  • at day service
  • in education settings
  • in the community
  • at respite
  • residential home
  • nursing home

We may also ask you to meet us at our offices.
