The Lead Professional handling the person’s care must make a referral once the person is registered on the Dynamic Support Register (DSR) by filling in a referral form.
When referring a person who is aged 13 or over please seek their consent. If a person is unable to consent themselves please seek parent/carer consent.
If you cannot access either of these forms, please contact us. If you are unsure whether a person is registered on the Dynamic Support Register (DSR) then please contact your DSR Lead: hnyicb.admissionavoidancehub@nhs.net
Dynamic Support Keyworkers will speak to you and your family or carers and agree the best place to meet.
Dynamic Support Keyworkers will work with you to identify parking and transport links.
