Frequently asked questions

Below you’ll find answers to the questions we get asked about the Dynamic Keyworker Support Service

How do you work with the Dynamic Support Register (DSR)?

Dynamic Support Keyworkers makes sure that important updates are shared to the Dynamic Support Register about people's needs, along with any relevant updates from professionals across health, social care and education professionals. This group of people work together to look at whether there is progress towards meeting people’s needs to try and minimise risk. The Dynamic Support Register Networks for each local area regularly meet to check that the right support is being given to people.

Who can make a referral to the Humber and North Yorkshire Dynamic Support Keyworker Service?

Referrals must be filled in by the Lead Professional handling the person’s care. However, we will seek consent from parents/carers as well as from the young person where appropriate.

Referrals will only be accepted if the person is identified on the Dynamic Support Register and risk rated either Red, Amber or Blue (inpatient).

Please see further information on the referral process section of this page.

Is the Humber and North Yorkshire Dynamic Support Keyworker Service only open to those who have a diagnosis of Autism, a Learning Disability or both?

Yes. We support Autistic people and/or people with a Learning Disability up to the age of 25 who are on the Dynamic Support Register risk rated Red, Amber or Blue.

Are you happy to receive enquiries to your service?

Yes. We are happy to receive enquiries to our service from young people, families and carers. We may be able to give advice and signposting. Please refer to the contact details section for further information on how to contact us.

How long does it take to receive support from the Dynamic Support Keyworker Service?

Once we receive a referral from the Lead Professional then this is looked at during our twice weekly referral meeting, which takes place on a Tuesday and Thursday. If we can offer support to the person, then with their consent, the Dynamic Support Keyworker will seek to find out as much information about the person’s situation as possible before planning a first meeting.

How long will you support a person?

A Dynamic Support Keyworker is allocated when someone is identified on the Dynamic Support Register as risk rated Red, Amber or Blue (Inpatient). The Dynamic Support Keyworker will work with people to make sure that they get the right support at the right time from services to meet their needs and lower the risk. The support is led by peoples individual needs but is time limited as the risk lowers.

Dynamic Support Keyworker make sure that the person has a personal plan, which can be shared with others about what matters to them and what support they need. This can be shared with other services to make sure that care continues once the support your receive from a Dynamic Support Keyworker comes to an end.

Do you give clinical support, such as counselling/Cognitive Behavioural Therapy?

No. Dynamic Support Keyworkers work with the person and their parent/carer (if relevant) and services to make sure people get the right support at the right time. We also support people and their parent/carer to be involved in how their care is delivered.

Do you provide advocacy?

We are not an advocacy service. However, we can give information and support with referral to local advocacy services. Dynamic Support Keyworkers will work to make sure that people’s and their parent/carers (if relevant) views are heard in meetings.

What does step down mean?

This is when support from the Dynamic Support Keyworker Service will come to an end. This will only happen when support from the Dynamic Support Keyworker has showed positive change and/or that the risk for people is lower, or where the keyworker role and the person's plan shows no further need from the Dynamic Support Keyworker. This will be a gradual process and the Dynamic Support Keyworker will make sure that support from other services continues.

Are people able to access the service again once Dynamic Support Keyworker support has ended?

We understand that people’s needs change. Dynamic Support Keyworkers are there when people’s needs and risks increase, but as progress is made and situations become settled, the Dynamic Support Keyworker steps down. People may still be on the Dynamic Support Register and will have continued support from their networks around them. People can also contact the Dynamic Support Keyworker Service through our enquiries line and or email.

Sometimes the situation changes and if this is identified to the Dynamic Support Register and the person meets the criteria for a Dynamic Support Keyworker then a new referral would need to be filled in by the Lead Professional to advise of the new situation and the changes.

Please see further information on the Contact details and referral process section of this page.

Are Dynamic Support Keyworkers able to help with practical matters such as benefits and housing?

Our Dynamic Support Keyworkers can help signpost and help you access the information that you need. They may also be able to help you with writing supporting letters and filling in forms.

Do you link with Local Parent-Carer Forums?

Yes. We link with all Parent-Carer Forums across Humber and North Yorkshire. We do not share any personal information with them, however they have been involved with developing the service and we continue to update them about the service.
