Aged up to 25 years of age. If you are aged 18 or over allocation of a Dynamic Support Keyworker will focus on the transition between children and adult services.
On the Dynamic Support Register rated either RED, AMBER, OR BLUE (inpatient). People who are identified as GREEN may receive advice and information.
Registered with a GP in North Yorkshire, York, Hull, East Riding, North Lincolnshire or North-East Lincolnshire.
