Hull and East Riding Long-Term Health Conditions Services (LTHC)

What we do

What we do

The Long-Term Health Conditions Team support children and young people until the age of 18 who have a diagnosed physical health condition.

Having a physical health condition can impact on how a person feels about themselves, whether they are able to do things they enjoy, and can cause difficult emotions. Below are some reasons we may be asked to support a child, young person and their family.

Adherence to treatment plans

Some children and young people may be prescribed medication or be on a treatment regime that they have to follow.

Sometimes a young person may struggle to manage or adhere to their treatment plan and this can result in poor health outcomes and concern among family members.

Our team can provide support to young people and their families to work through any barriers there may be to treatment adherence. This can also include worries about certain medical procedures the young person may need.

Adjustment

When a child or young person first receives a diagnosis of a physical health condition this can bring about huge life changes for them and their family.

It may feel confusing, hard to make adjustments, and may bring up lots of difficult feelings. Children and young people may show these feelings by their behaviour. Families may come to us for support around this adjustment.

Emotional wellbeing

Having a physical health condition can be difficult. How people feel about their physical health condition can impact on other areas of their lives; how things are going in other areas of their life can impact on how they feel about their physical health condition.

Some children or young people may feel angry, sad, worried, frustrated or confused. Some people might feel all of the above feelings, or a mixture of these. How we feel can affect people around us too, so we provide support to young people and their family to manage these emotions that arise as a result of the physical health condition.

Frequency of contact varies between weekly and fortnightly. The length and frequency of appointments can be flexible to what works best for the person. Sometimes we work with somebody 1:1, and at other times we might work with families and/or carers as well as health care professionals. You may decide you would like someone else with you to support you.
