How to access the service

The IST accepts both internal and external referrals from services, as well as self referrals and referrals from family members.

The IST accepts both internal and external referrals from services, as well as self referrals and referrals from family members.

Most referrals to the IST are internal referrals and come from the Community Team for Learning Disabilities (CTLD). This is usually when it is felt that the person’s needs require more intensive support at that time.

Internal referrals are discussed at a weekly staff team meeting and are allocated to two members of the team to complete an ‘IST assessment’. This should happen within 7 days and the aim is to decide whether IST needs to be involved at this moment in time. This assessment should involve the people who are part of the service user’s care and is a conversation about what the current needs are.

External referrals tend to come from care providers or those working alongside the service user. However, we also accept referrals from services users and family members.

Criteria to receive

  • A person has a learning disability, and their behaviour presents with such frequency and intensity that intensive support is needed to prevent placement breakdown or an admission to hospital
  • We will also consider referrals where the person requires a consistent and intensive multi-agency approach, but they have disengaged from services
  • We will also consider referrals if the person is not under a community forensic team, and they pose a risk of offending. In these cases we might offer a longer term piece of preventative work

The decision as to whether somebody meets the criteria for the IST is based on the assessment process.
